A bottle of prescription medication was observed sitting on the dinning room table accessible to children. Note: The caregiver removed the bottle from the table and ensured it was put up where the children could not access it.

The TV was on when I arrived for my inspection and a child under two years old is currently present and was watching the TV.

There are over head rings on one of the swing sets.

The admission forms do not have the completed enrollment agreement for the children.

The see saw is in the use zone of the swings.

There is a see saw attached to the swing set. There is also a see saw on the playground with four seats.

The operation was providing childcare services for 6 unrelated children. The care being provided met the definition of regular care.
