Three infants did not have an infant feeding instruction form.

The food allergy emergency plan for two children was not signed by the parent. One food allergy plan did not indicate the symptoms and the steps to take if the child has an allergic reaction.

There was no activity plan for the Prek group. The activity plan for the toddler group was not dated. Note: A new activity plan for the week was posted for the toddler group.

There was no Emergency medical transport and treatment authorization form for a child who has been transported from school to the child care operation. In addition, there was no information of the parent's name and emergency telephone numbers. Corrected at Inspection: Information was placed in the transportation binder.

Loose fill surfacing (pea gravel) was not maintained at the proper depth for a play structure in the outdoor play area. In addition, the depth of the pea gravel was not marked.

The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

A 4-month-old infant was asleep in a restrictive device, swing. Note: The caregiver removed the infant from the swing and placed them in the crib.

Two classrooms were observed with at least 4 ceiling tiles with holes and water stains.

A child was left behind in the playground area unattended for an undetermined time after other children were transitioned back into the classroom.

A review of the Employees Who Left Employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

A caregiver went on route that did not have current transportation training.

2 out of 8 children files reviewed were missing immunization records.

2 out of 8 employee files reviewed were missing trainings. They are as follows: 1) Abuse/Neglect, Emergency Preparedness, Communicable Diseases 2) Food Allergies

A timely inspection was not completed in 2023. Fire inspection was last completed on 8/22/2022.

Two caregivers that transport children in the operation's vehicles do not have transportation training.

A play equipment was observed in the use zone of the exit of a slide in the playground area. The 6 feet use zone was not compliant. NOTE: The use zone in front of the exit of the slide was made compliant during the inspection. Play equipment was moved away from the 6 feet use zone of the slide equipment.

Infant group was excluded from practicing a fire drill during the month of August and September as per a caregiver and director of operation. The last completed fire drill was for the month of September 2023, but infants did not participated in the drill.

Admission information for a child was not available for review during the investigation inspection.

Based on evidence gathered during the investigation, use of prohibited discipline and guidance methods were practiced at the operation. An employee pulled a child to redirect, and a second employee slapped a child's hand/arm.

One caregiver was missing 3 hours of pre-service training to meet the requirement of 8 hours prior to being counted in child/caregiver ratio. A second caregiver was missing the orientation statement.

One caregiver was providing care of 6 infants with a specified age group of 0-11 months old. Child/Caregiver ratio was deficient. NOTE: This was corrected during inspection. A second caregiver was added to the classroom.

1 out of 8 caregiver records checked did not have the following trainings available for review for training year 2021-2022: -Emergency preparedness -Medication Administration

As per caregivers, cloth toys in the infant classroom are being machine washed every two or three weeks. Two of the cloth toys were observed to be in use during inspection.

Children's products certification was not completed timely. It was last completed on 8/31/2020.
