Director did not have a time sheet/attendance for the hours worked in the child care operation. Corrected at Inspection: A time sheet was completed for the director for the current and past week.

1 out of 6 children's records reviewed is missing immunization records.

Fire drill conducted on 10-2-24 had an exit time of 5minutes

The outdoor play area was observed with red ants. There were two houses with chipped paint, damaged wall, and exposed nail.

Three cribs used by infants were not labeled with their name. Corrected at inspection: The labels were placed in the cribs.

Three infants did not have a current feeding instruction.

The two transportation vehicles were observed with pieces of papers, trash and candy wrappers. Corrected at Inspection: The vehicle was cleaned during the inspection.

There was no emergency medical transport and treatment authorization for a child that is transported from school. Corrected at Inspection: The child information was placed in the vehicle before transportation.

A child's record did not have a copy of the immunization record. A child's record did not have a health statement on file.

Caregivers in three different classrooms did not wash their hands after wiping children's noses. NOTE: This was corrected by caregivers washing their hands after inspector informed them about standard.

1 caregiver was providing care for 6 infants with a specified age group of 12-17 months old. Child/Caregiver ratio was deficient by 1 child. NOTE: This was corrected during inspection as another caregiver was added to the classroom.

The carbon monoxide detector was not working properly as observed during inspection. NOTE: This was corrected during inspection. A working carbon monoxide detector was added during inspection.

Training hours for training year 2020-2021 were not available for review during inspection for Director of operation.

Children's products certification was not completed timely in 2021. Last completed certification on 3/2/2020.

School age children were watching a TV show on a tablet while eating their afternoon snack. NOTE: This was corrected. The tablet was removed by caregiver.
