Operation is using state admission form, however state admission form is not current.

Three children were missing the emergency contact address on the admission form.

During the inspection, the diaper changing pad in the infant room was observed with a hole in it.

The admission form was not signed or dated by a parent for a child in care.

During the inspection, the emergency practice form for fire drills was observed and missing March 2024 dates.

During the inspection, in the infant room there were chemicals such as roach spray and Zevo insect plug ins on the floor.

During the inspection, there were three training cups and one bottle not labeled with the children names.

During the inspection, the fire extinguisher in the van was observed with the needle in the red on recharge. Fire extinguisher has not been inspected by fire marshall since 2023.

During the inspection, it was observed a 4 month old child observed in the crib with a stuffed animal.

A child was missing the address for physician information documented on the admission form.

Carbon monoxide detector has not been checked since February 2024.

During the inspection, the emergency practice form for lock-down drill was observed and missing dates March 2024.

During the inspection, the binder in the van was observed not to have emergency medical information for children being transported.

Operation could not provide proof of current liability insurance.

Staff hired on 1/23/2023 has completed 8 hours of pre-service training. Staff is missing the remainder 16 hours.

During time of inspection, Fire inspection was conducted on 4/26/2022.

During time of inspection, one child did not have immunization on file.

During time of inspection, two caregivers were observed using their personal phones during naptime.

During time of inspection, one child did not have food allergy plan on file.

The health statement section of the enrollment form was left blank and not completed by the parent.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Two employees had an affadivit that was filled out, but not notarized.
