One of the four caregivers' records reviewed did not have the required 21 training hours for the last full year of employment.

Emergency medical transport and treatment authorization forms for each child being transported were missing for all the children being transported. Director made copy of all the admission form for the children being transported and added to the transportation binder.

Six out of eight childrens record review were missing current immunizations records.

The center does not have a recent sanitation inspection, the last sanitation inspection expired 9/16/2024.

Upon reviewing the personnel files 2 caregivers were missing their -2021-2022 (24 hours of training requirements).

The glass in the sliding door of the infant room classroom needs to be marked with decals or other marking at child's eye level. The outlets in the toddler classroom need to be covered. Chlorine wipes in the after school classroom need to be moved to a higher shelf.

The first aid kit in the transportation vehicle is missing: cotton balls, a guide, thermometer, tweezers, and scissors. The first aid kit inside the building is missing tape.

The fence in the playground needs to be repaired. There are missing/broken slats. Part of the fence is unstable/leaning out. There is also a large tree limb in the playground which needs to be removed. Tiles are missing in the floor of the pre school classroomm.

One of the restrooms in the hallway is missing paper towels.
