Of the 10 children's files reviewed, three did not have the Date of Admission documented on the Admission Form. This was corrected during the inspection when the director documented the Date of Admission on the Admission Form.

The operation failed to obtain a signature from the parent once the incident report was completed.

The operation allowed one person to be present and have unsupervised access to children without submitting or receiving the results of an initial background check and FBI fingerprint.

Inspector observed one staff with more than 26 children while in the cafeteria. Inspector also observed one staff with approximately 36 children outdoors. A second staff had children outdoors in her area with a total of 41 children in care between the two staff. This is a re-cite.

Three children with food allergies did not have a food allergy plan on file.

Inspector observed one staff with 46 children on two different occasions during the inspection. Inspector also observed another staff with over 35 children during the inspection.
