Ten children's files were reviewed: one file is missing updated immunization records, second file is missing health statement and updated immunization records and a third file is missing health statement and consent/release information.

Two out of eight staff files reviewed are missing notarized affidavit (form 2985).

Ten children's files were reviewed. Two files are missing health statements. One file is missing emergency contact address, releases information and special care information. Seven out of the ten files reviewed are missing the updated information regarding special care.

One caregiver was supervising 17 children ages 3-4yo, SAG 3yo. Classroom was over ratio by two children. One caregiver was supervising 12 children ages 18mo-2yo, SAG 2yo. Classroom was over ratio by one child.

Operational policies were reviewed, and the following information is missing: 18(a) The benefits of physical activity and outdoor play, 18(d) Each setting in which your physical activity program will take place, 18(f) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play and 18(g) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play. (30) Procedures for supporting inclusive services to children with special care needs.

Nine staff files were reviewed. One file is missing training certificates on the topics of hazardous materials, food allergies, administering medications, emergency preparedness and communicable diseases. A second file is missing copy of driver's license for a caregiver that participates in transportation. A third file is missing copy of foreign high school diploma or equivalent.

Most recent fire inspection provided is dated 3/18/22 and a renewal inspection is due every twelve months.

Most recent sanitation inspection provided is dated 3/03/22 and a renewal inspection is due every twelve months.

There was no unitary surfacing material for a slide requiring surfacing in one of the children's outdoor playgrounds. The slide was placed over concrete and measurements were 22 inches in height. Note: This was CORRECTED during inspection as the slide was removed from the children's playground.

An infant was observed asleep with a bottle in a crib.

1 of 4 children's files reviewed was missing the child's immunization record.

1 of 4 transportation vehicles was not maintained. The side of the middle seat was missing a plastic cover. This posed a safety hazard as screws that had sharp edges were exposed and children could accidentally scratch their legs as they try to access the back seat of the van. Note: The director temporarily covered the exposed screws with duct tape during the inspection.

1 of 4 transportation vehicles was not equipped with a fire extinguisher. Note: This was CORRECTED during the inspection as a fire extinguisher was obtained and placed in the transportation vehicle.

A transportation vehicle designed to seat eight persons was not equipped with an electronic child safety alarm.

3 of 7 personnel records reviewed did not have the caregivers education documentation translated into English for education received outside of the United States.

The emergency medical transport and treatment authorization forms for 24 children being transported were missing from the transportation binder. Note: This was CORRECTED during the inspection as the required forms were obtained and added in the transportation binders.

A caregiver was observed caring for 8 infants during naptime. The specified age of the children in the group was 12-17 months of age. According to the ratio chart the maximum number of children one caregiver may supervise is 5.

There was more than an 18 month difference observed in the infant room when a toddler was combined with the infants during naptime. The youngest child was 6 months old and the oldest child was 29 months old.

An employee was allowed to be present at the operation and have direct access to children prior to receiving the employee's eligibility status from the Centralized Background Check Unit.

A caregiver was observed caring for 7 infants. The ages of the children in the group were a 5 month old, 11 month old, 14 month old, 15 month, 16 month old, and two 17 month olds. According to the ratio chart the maximum number of children one caregiver may supervise is 5. The group was out of ratio by two children.
