LaLa's Home Daycare

The primary caregiver did not report suspected abuse of a child in care. The primary caregiver noted serious concerns regarding bruises that were observed on a child in care and stated that she knew she was required to report the suspected abuse. This information was obtained through the course of the investigation.

The daycare home was operating over capacity. A video was provided as part of the investigation and there were at least 13 children observed to be in care. Thirteen children can clearly be seen in the video and it is unclear if a fourteenth child is laying in a bouncer, on the floor, near the table.

Two infants, that were not able to sit unassisted, were observed to be fed in bouncer seats. They were not being held by the caregiver as required. This was observed through video obtained during the course of the investigation.

A caregiver was observed placing infants in highchairs without using the safety straps. This was observed through video obtained during the course of the investigation.

The primary caregiver did not sanitize the diapering surface or use a clean disposable covering in between each diaper change. This was observed through video obtained during the course of the investigation.

An assistant caregiver placed a crying child in time out in a play yard for 6 minutes. The primary caregiver then moved the play yard with the child inside of it to a bedroom. The child was alone in the bedroom with the door shut for 9 minutes. This was observed through video obtained during the investigation.

-A caregiver did not use good judgement when placing two infants on top of a table, in bouncer seats when feeding them. This was observed through video obtained during the course of the investigation. -A caregiver did not use good judgment when picking up a child up by the wrists and placing them inside of a play yard. This was observed through video obtained during the course of the investigation

The primary caregiver yelled into a crying toddler?s face while the child was standing in a play yard. The child fell back into the play yard after the caregiver yelled at them. The primary caregiver then yelled at the child and told them they would not eat if they did not stop crying. After removing the child from the play yard, the primary caregiver told them they would go back in the room if they did not stop crying. This was observed through video obtained during the course of the investigation

The play yard mattresses were covered with a thin regular sheet and 2 quilted sheets under the thin sheet.

The portable cribs had a mesh liner surrounding the outside of the crib slats. The mesh was woven between the slats and secured with zip ties on the outside of the crib.

An infant feeding plan was not followed when the provider fed an infant an additional 2-4 ounces of formula on multiple occasions. The infant feeding plan provided by the parent instructed the provider to prepare 4oz every 2.5-3 hours.

During the course of this investigation it was determined that the provider continued to use the extra mattresses in the play yards after being cited on 8/13/21. On 8/27/21 a follow up inspection was conducted by Child Care Regulation. The provider admitted to parents that extra mattresses continued to be used after she was cited for using them, and they were removed prior to Child Care Regulation entering the home.

Infant feeding schedules are not being reviewed, updated and signed every 30 days. Feeding instructions are provided upon enrollment and not updated and signed by the parent every 30 days.

The primary caregiver did not use good judgment when she fed an infant formula that the parents asked her specifically not to use. The provider also used a faster nipple for the infant's bottle than provided by the parent and without permission.

During the course of this investigation is was determined that an infant was fed a bottle while laying flat on their back causing them to choke on the formula.

Based on the seriousness of the deficiencies cited in the investigation, it was determined that the permit holder is not ensuring that she is running her home in compliance with the Minimum Standard Rules.

This standard is being recited as the pack n plays are still being supplemented with mattresses that do not follow manufacturer instructions nor were they designed by the manufacturer.

A household member did not have complete background check and has not received the eligibility letter.

During the administrative review process the original citation, HRC42.04412(a) - Interference with an Investigation, was overturned. This standard for making records available during an inspection is more specific and details the records that were not made available to the inspector upon request.

During the 07/27/2021 inspection, I observed two mattresses were placed in a Graco play yard. One was a Dream on Me mattress and one was a thinner mattress. The manufacturer instructions state you must never supplement the Graco Play mattress pad with other mattresses, pads, or pillows.
