The operational policies were missing the following; (15) water activites, (17) animals, (18) duration of indoor and outdoor physical, (19) Procedures for providing and applying sunscreen and bug spray. (24) the emergency prepardness plan, (30) Procedures for supporting inclusive services to children with special care needs.

It was determined that the younger infant classroom has not being going outside.

It was determined that the operation has 18months to school age combined past the 45 minutes allowed for opening time.

It was determined that a caregiver was unaware of the ages of children in their care during opening hours.

Two staff are missing a signed statement that they received the operational policies.

Six staff are missing the annual 24 hours of training for the last full year.

Three staff are missing pre-service training.

The two playgrounds do not have 6 inches of mulch for surfacing.

Two staff are missing signed statements that they have completed orientation.

Three children files are missing the addresses for the emergency contacts listed.

Eight children files are msising the special care statements updated.

Three staff are missing the Pre-Employment Affidavit.

Two staff are missing the Notarized Affidavits.

One staff is missing first aid training.

One of the vehicles being used does not have an alarm installed.

Ten childrens files were observed missing the updated water activity information.

Ten childrens files were observed missing the updated special care statements.

The following is missing for the staff training hours from the last full year; -Two staff are missing training inEmergency preparedness. -Five staff are missing training in Preventing and controlling the spread of communicable diseases. -One staff is missing training in Administering medication. -Three are missing training in Preventing and responding to emergencies due to food or an allergic reaction. -Six staff are missing training in Understanding building and physical premises safety. -Two staff are missing training in Handling, storing, and disposing of hazardous materials.

One staff files was observed missing a notarized affidavit.

Three infant feeding instructions have not been updated since May 2024.

One childs food allergy form is not signed by the doctor.

Four children files were missing health statments.

Three classrooms had broken outlet wall plates.

One child's epiPen that is on file is expired.

Four staff are missing the annual 24 hours for the last full year.

Two staff are missing Abuse and Neglect training for the last full year.

The older children's playgrounds mulch measured at 4 inches. The younger children's playground mulch was measured at 3 to 4 inches.

Nap cot procedures are not being followed in two classrooms. Children are not getting their assigned cots.

Nap cot procedures are not being followed throughout each classroom. The children's nap cots in the classrooms were labeled with numbers, but there is not a system in place where children will get the same cot everyday.

An infant was observed awake in a crib, in a sleep sack swaddle. The swaddle type was wrapped around the childs torso. The sleep sack swaddle was removed off of the infant.

The fire inspection was not available for review during the inspection.

The bus binders were evaluated and 4 children do not have infromation for parent s names, telephone numbers, and emergency telephone numbers.

One childs medication on file was observed and was expired.

On the open grass play area, there were two broken toy cars.

The older childrens playgrounds mulch measured at 2 to 4 inches. The younger childrens playground mulch was measured at 2 to 3 inches.

The bus binders were evaluated and seven children do not have emergency medical transport and treatment authorization information.

The 4 year old and older playground does not have the required 6 inches of mulch. Some areas have 3 inches and one area has 5 inches.

Two staff were present at the operation caring for children in classrooms, and did not have an active background check on file.The backgrounds checks were ran while at the operation and the two staff were sent home.

The 4 year old and older playgrounds mulch measures at 1 inch to 3 inches. At one part of the playground the liner is exposed.

Based on evidence obtained, children are not supervised adequately on the playground resulting in an injury that required medical attention. Observations made during the investigation inspection showed caregivers not intervening as children inappropriately used playground equipment.

3 infants were observed swaddled while napping.

6 of the 11 employee records were not current on their annual training.

2 children were missing health statements.

Several classrooms were observed to uncovered electrical outlets.

Several bottles and training cups were not labled with infants'/toddlers' names.

Several infants were observed with unfastened safety straps when sitting at their tables.
