The feeding instructions for one infant have not been updated within the last 30 days.

On the playground the following was observed: Several Broken little tyke cars were observed on the playground. A tunnel toy was observed with the tunnel part caved in and broken. The slide on the large play structure was cracked and broken at the exit of the slide. The small play structure had rusted platforms. The tick tack toe toy had several cracked and broken parts. The yellow truck stationary equipment had paint peeling.

Two classrooms had paint chipping on the walls.

The loose fill in the use zones of both climbing structures needs to be maintained as there is grass and weeds growing through the loose fill. Also the loose fill needs to be maintained to ensure the 6 inches of depth.

A caregiver was observed using their cell phone while responsible for supervising children in care.

In the infants, wobblers and 2's classroom the changing table mat had rips in them. Photos were taken.
