One child with a food allergy plan had an epi pen that expired in July 2023. One child that has an inhaler had the medication for it that expired April 2023 and January 2024.

The operational policies have not been updated to include parents rights as well as the promotion of indoor and outdoor physical activity.

The last conducted health inspection was 9/4/23.

The operation did not notify CCR about the playground undergoing construction and not being used and that a temporary outdoor play area was set up. Note: This was corrected during the inspection when notification was given and the information was reviewed.

It was observed there was not a promotion of indoor and outdoor physical activities section in the operational policies with all the required components.

One of the two's room with a diaper changing changing station does not have a sink. The other two's room has a diaper changing station with a sink but it's not in the classroom and is in the bathroom of the classroom.

I observed 4 of 6 personnel files did not have documentation of meeting the educational requirement. One of the files missing the documentation was provided during the inspection.

There was not documentation of the required annual training hours for one of the directors.

3 of 11 children's records evaluated did not have an address for the emergency contact(s). 3 of 11 children's records evaluated did not have the date of admission. Note: The dates of admission were added to the files during the inspection.

In 6 of 11 children's files evaluated there was no written acknowledgement that he parents received the operational policies.
