One caregiver had 12 children in care. The specifed age of the group was 2 years old. One caregiver is allowed to care for 11 children with the specifed age of 2 years old. The classroom was over ratio by 1 child.

The diaper changing pad in the infant room had a large tear. The large tear makes the changing pad absorbent.

The anchoring devices under two spring loaded outdoor play equipment were exposed.

The side walk outside on the playground had water constantly dripping on it. The dripping is from a leaking faucet. This sidewalk is accessible to children which poses a slipping hazard.

The gym floors had chipped paint from the falling off the walls. In the wobbler room the handwashing sink had black slim from food being placed in the sink. The trash can in the wobbler room had exposed hard food on the lid. The floors through out the facility had not been vacuumed or cleaned.

Multiple crib mattress being used in the infant room were not waterproof or machine washable.

A 3 month old infant was observed sleeping on their stomach on the floor.

Two infants were observed in their cribs with bottles. One had fallen asleep with the bottle still in their mouth, the other was still drinking the bottle with their eyes closed.

The door in the gym had two large holes. The second door in the gym was observed dirty. The ceiling tiles in the gym and room 1 are observed broken and missing.The walls throughout the facility have chipped paint. One wall is lacking sheet rock. The baseboard in room 1 on the sink is hanging off.

A caregiver did not wash an infants hands after their diaper was changed.

The pea gravel that is by the swings was not the required 6 inches.

The fencing is covered in weeds. At the bottom area of the fence outside there are large gaps where children could crawl under. The fencing outdoor is pushed over due to large limbs and brush. There is a deep whole in the ground by the sidewalk. A toilet seat is missing from one toilet.

The side walk where children walk had standing water and algae where children could slip and fall.

An infant was observed sleeping on a boppy pillow.

The operation did not have a current fire inspection.

During the course of the investigation, it was determined that the operation exceeded maximum group size.

The operation did not notify all parents of children enrolled of a safe sleep violation. The operation received a safe sleep violation on 5/12/25. The operation had until 5/17/2025 to report the situation to parents and the operation did not report the violation to parents.

An infant younger than 12 months of age, was observed sleeping in a crib with a pacifier and a pacifier clip attached.

An infant was observed sleeping in a restrictive device.

The infant room diaper changing pad had a tear which makes the diaper changing pad absorbent.

The floors, walls, and bathrooms were observed to have standing dirt. The ceiling tiles in the gym were observed missing, broken, or torn.

The bus used to transport children did not have a safety alarm installed.

One caregiver was observed with 5 infants. The specified age group was 0m-11m. The amount of children on caregiver can supervise at one time is 4. The caregiver was over ratio by one child. Note: This was corrected when a child was moved to another age appropriate classroom.

The diaper changing pad in the toddlers room had a large tear.

Three infants were observed in restrictive devices for more than 15 minutes.

The school age room and the prek room did not have the activity plans posted for parents to see.

The carpets in the classrooms had not been vacuumed. The floors in the classrooms had food stuck on them and the baseboards were black in some spots. The bottom of the toilet in the toddler room had not been cleaned.

Three out of ten children files reviewed lacked current immunizations.

A child was observed in a highchair not buckled in.

The vehicle used to transport children, registration sticker expired in August 2023.

One out of ten children files lacked a current immunization record.

One potty chair had not been sanitized after being used. Note: This was corrected when the potty chair was sanitized.

One child lacked a food allergy plan on file.

One out of ten children files was lacking current immunization record.

Two doors and one wall in a bathroom were observed to each have a hole in them (ranging from 1/2 inch to an inch in diameter). Note: A temporary fix (tape) was placed over the holes to reduce the risk to children.

One infant feeding instruction has not been updated within the past thirty days.

Four infant chairs being used by children were not equipped with safety straps. Note: The chairs were removed from the classroom until safety straps can be purchased and installed.

The consumer product recall has not been certified in the past 12 months.

The operation failed to notify licensing of two communicable illnesses (COVID-19) within the required time frame.

A caregiver was observed holding the door to the playground open and allowing two school age children to go inside to use the bathroom alone. The other children remained on the playground.

The toddler class sat at the table for approximately ten minutes with no activity while the caregiver conducted diaper changing.

A caregiver failed to wash a child's hands after changing the child's diaper.

The floor in the infant room and toddler bathroom was sticky and visibly dirty. A white and yellow shelf in the toddler room was visibly dirty and had crayon marks. An outdoor faucet leak on the playground posed a hazard as it was above the sidewalk, causing it to be slippery.

Hooks that were being used for children's belongings were not labeled.

A child under the age of 2 years old was observed having screen time.

Peeling paint was observed in the school age classroom, posing a health hazard.

The fire extinguisher in the vehicle used for transporting has not been serviced since February 2020.

Three active ant piles were observed on the playground.

Condition #6 requires that all new staff hired during the probation period must be notified of the probation and conditions. There also must be documentation of what was discussed and a signature. There was no signature for a caregiver that was hired on 7/28/2021. Note: This was corrected when the caregiver provided a signature.

The school age bathroom floor was visibly dirty, the sink area was also dirty and a roach was observed. The baseboard area in the classrooms are visibly dirty.

Several bottles and cups in the infant room were not labeled.

The director and a caregiver lacked current transportation training.

An infant caregiver lacked current SIDS/Shaken baby training.

One toilet seat was damaged, carpet in the toddler was dirty, floors were observed dirty/sticky,gym floor is dirty/stained, bathrooms smelled of urine, and paper towel holders/sinks were dirty.

A caregiver failed to wash the child's hands after changing the child's diaper.

Grass on the playground exceeded six inches in some areas, an active ant pile was observed, and animal feces.

The after school children were sitting on the carpet for at least ten minutes with no activities planned or offered.

A caregiver failed to wash her hands after changing a child's diaper.

The toddler room had seven toys for twelve children to play with.

The diaper changing table was not sanitized after the caregiver changed a child's diaper.

A caregiver was observed holding a four year old child by the arm with both of her hands. He was fighting and pulling trying to get away. The caregiver then threatened to have the parent take away karate from the child.

Operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility. The staff member was supervising children in the infant room along with another caregiver in the infant room with a cleared background check.

Caregivers stated there were sixteen children in care, however there were seventeen children in care. One child was not accounted for.

The operation failed to contact EMS after a child received a significant injury while in care.

The operation failed to notify the parent that their child was seriously injured while in care.

The operation failed to supervise the children, which led to a child receiving a serious injury. This injury may have been prevented if the child was being properly supervised.

A caregiver was neglectful when the caregiver failed to intervene when a child was playing by a cabinet with the hook. The child then hung on the hook with his teeth and eventually lost grip. This led to the child receiving a significant injury to the face. If the hook would have gone through the child's palate inside the mouth, eye, or inside a nostril, it could have potentially caused more physical damage to where more intensive medical attention would have to be warranted.

The operation failed to provide proper First Aid to a child after the child received a significant injury.

Infant room was not in ratio with five infants under the age of 10 months and one caregiver supervising the children.

Through information received with this investigation, it was determined not all parents were notified of the lice outbreak. The parent notification form was not accessible and not publicly accessible to all parents as the notification was posted on the interior side of the front door facing inward. Parents, at the time, were not allowed inside the operation due to the COVID-19 Emergency Orders prohibiting parents entering the operation not allowing parents immediate access to the notification.
