Self-Reported: During investigation it was found, two children were able to open the door and go onto another classroom and were unsupervised for approximately 8 minutes.

During inspection, one vehicle's transportation van lacked the required thermometer and scissors in the first aid kit. Note: This was corrected during inspection when director added the thermometer and scissors to first aid kit.

During inspection, one of the fire extinguishers was last serviced 03/2024.

It was found during an investigation that a child was left unsupervised for less than 5 minutes in a classroom.

Two out of ten files reviewed did not have the authorization for emergency medical attention completed and signed.

Three out of the five written feeding instructions reviewed were not signed by parent or updated signature by last 30 days.

During the investigation, it was determined that the operation failed to notify CCR of an incident that placed a child at risk.

Based on interviews conducted it was determined that the caregiver in the two-year-old classroom raised her voice to a child in a harsh manner.

Sufficient evidence confirmed that a two-year-old caregiver grabbed and pulled a child's forearm leaving a bruise.

There was no incident report created within the adequate timeframe and the parent was not given a written notification on the details of the incident.

Four out of eight feeding forms were found not updated. Forms were signed and dated as of April 2023. One child's feeding form was not ready for review.

One children's files reviewed did not have a current immunization record. Note: The immunization record was obtained during the inspection.

Three classrooms were found with electrical cords hanging at children's reach.

School age children were found laying down right next to each with no space between them.
