After diapering, gloves were utilized, a caregiver was observed to not wash their hands after removing their gloves and was going to continue changing additional changing. Noted: This was corrected during the inspection when the caregiver was advised to wash their hands after removing gloves and in between each diaper change.

After a diaper change, the caregiver was observe to not sanitize the changing table surface and was going to continue changing another child's diaper. Noted: This was corrected during the inspection when the caregiver was advised to sanitize the surface and reminded to sanitize between each diaper change.

Four of the ten children's files reviewed did not have current immunization records in their files available for rreview.

The loose-fill on both active play spaces was observed to not be installed to the required depth of six inches around and under the play structures.

The operation has not had an updated fire inspection. The most recent fire inspection was completed on August 6, 2024.

A caregiver failed to evaluate a child's ear after being notified of a bead being inserted into the ear.

Cleaning supplies were accessible to the children in care as they were located in the cabinet beneath the diaper changing table with the child safety lock unlocked. Note: This was corrected at inspection as the child safety lock was locked.

A caregiver did not sanitize the diaper changing table after a diaper change. Note: This was corrected at inspection as the caregiver sanitized the diaper changing mat.

A fire inspection was not conducted at the operation within the past 12 months. The last fire inspection was conducted was on 1/28/2020.

The operation did not document the monthly fire drills and carbon monoxide detectors test for the previous three months.

Six out of ten records reviewed at the operation did not have a health statement. Six out of ten records did not have updated immunizations.
