One caregiver did not have current training in pediatric first aid. The caregiver is scheduled to take the course on 9/4/25.

One infant was sitting in a high chair without the safety strap fastened. This was corrected when the caregiver fastened the safety strap.

One vehicle had seat cushions that had rips and tears. Some of the vehicle's seatbelt mechanisms were missing the plastic piece that attaches to the back of the seat. One seat was repaired with duct tape around the area where the seatbelt comes out of the seat.

One child did not have updated immunizations. The child has an appointment scheduled for 1/20/25 to receive the required immunizations.

One caregiver with direct access to children was present without an eligible background check. This was corrected when the caregiver left the operation during the inspection.

Five children records did not include a signed Parent's Rights Form 2987.

Five out of ten personnel files were missing a notarized Affidavit for Employment.

Two children were missing updated immunization records.

Name to face checks are not being conducted consistently at 10,12,2,and 4 during the day, and cleaning checklists for the infant and pre-k rooms are not being completed daily.

During the investigation inspection walkthrough, the following things were observed. - the entry way floor and classroom floors had trash particles and spill stain discoloration - two garbage cans did not have lids, exposing the garbage to the children, - classrooms, bathrooms, and hallways had peeling paint - multiple tables were not sanitized after feeding leaving crumbs and other food particles on the table - used toilet paper was found on the edge of the toilet seat and on top of the toilet paper dispensers These things do not assist in maintaining the building and therefore does not protect the health of the children in care.

Children were observed watching television and eating breakfast. The caregiver was informed of screen time requirements and the television was turned off.

The operaton has paint peeling on several walls.

The restrooms were not cleaned and sanitized.

It was found that a caregiver utilized prohibited punishment on a child in care.

The operation did not notify parents after children were left unsupervised in a classroom.

A child was allowed to go from the playground inside the classroom unattended to use the restroom. Three other children followed this child inside. The children were unsupervised for approximately ten minutes. During this time two children were running with safety scissors in their mouth and the children were going in between different classrooms. Children are allowed to go from the playground inside the classroom unattended to use the restroom.

The operation did not report to Child Care Regulation within the required 2 days when three children were left unsupervised.

A caregiver was instructed on what needed to be said to the Child Care Investigator.

Caregivers yell at children as a form of discipline.

A caregiver was observed telling a child they would not get their food if they did not sit in the chair. Caregiver was asked to leave for the day.

During the investigation it was determined a caregiver yelled and humiliated children. The caregiver was immediately terminated.

There is enough evidence to support that a caregiver was not sufficiently intervening when children in care were interacting in rough manners with each other such as dog piling and wrestling with each other.

There is enough evidence to support that a caregiver was telling children to hit other children back when children hit each other.

There is enough evidence to support that caregivers were yelling at children in care.

There is enough evidence to support that a caregiver was grabbing and pushing children in care.

Based on information obtained during an investigation, it was determined that the childcare facility did not notify Licensing within the required timeframe regarding a situation that placed a child at risk when a caregiver grabbed a child by their shirt.

This standard was evaluated during the investigation and found out of compliance. A child was left unattended on the playground. The standard was marked into compliance at the time of the inspection when all of the caregivers were observed supervising in compliance with minimum standards. The director stated the caregiver who left the child unattended was counseled and retrained on supervision.

The fire inspection was not avaiolable for review.

A sanitation inspection was not available for review.

A plastic slide that was on the playground was broken causing a shear point in the slide.

There was no fire drill documentation for December or January. A fire drill was completed during inspection.

It was discussed with the director that tears in the vinyl are not able to be sanitized properly. This was corrected during the inspection when the operation covered the vinyl with tape.
