During inspection two employees were present supervising children that did not have an eligible background. The employees left during the inspection.

During the inspection, two of the five files were not available for review.

Out of 10 staff records reviewed the following was missing: - 9 records were missing annual Administering Medications training - 1 record was missing current Transportation training

One caregiver has a pending background check while caring for children. This was corrected at inspection when the caregiver left the classroom.

While conducting an investigation it was found that children were not appropriately supervised when the caregiver did not adequately intervene when a known biter was in the classroom.

Based on information obtained during an investigation, it was determined that a child was inappropriately supervised while in care.

Out of ten personnel records reviewed, five records did not have personnel and training records available for review.

Out of ten personnel records reviewed, five records were missing required documentation.

Out of ten children's records reviewed, four records were missing current immunizations.

A fire inspection had not been conducted within the required time frame.

Out of nine personnel records reviewed, the following was observed: -Three records were missing Proof of Education; -Two records were missing current CPR and First Aid certification; and, -One record was missing a Notarized Affidavit of Employment
