It was found during the DFPS investigation that caregivers are putting children on the porch and in the kitchen for discipline while the caregivers remained in another room. It was also found that caregivers let an 8 year old walk around the center unsupervised by staff.

Two caregivers were missing annual training on administering medications, allergy emergencies, and handling hazardous materials.
