Students were observed having screen time during snack. Note: This was corrected when the television was turned off.

One record did not contain the emergency contact address.

One student record did not contain the required health statement. The student was enrolled 1/6/2025.

Four out of four buses did not contain the emergency telephone numbers for each child transported.

Four out of four buses did not contain the emergency medical information for students transported.

One caregiver was observed in the infant classroom prior to receiving background check eligibility.

Based on the information obtained during the investigation, there is sufficient evidence to support a student was denied early childhood intervention services.

The gate located on the playground was observed with holes causing hazard to children. Note: Pictures were taken and this was corrected during inspection.

During the walkthrough of the operation, three classrooms did not label the children bottles with the child's first and last name. This was corrected when the caregivers labeled the bottles.

During the investigation inspection a staff member was observed caring for 12 toddlers without a cleared background check.

Based on the information obtain during the investigation it is determined that the operation caregiver used an inappropriate method to discipline a child in care.

During the walk through it was observed that the diaper changing pad had wear and tear. Note: this was corrected at inspection when it was replaced.

During the walk through two classrooms observed had chemicals (bleach bottle) accessible to children. Note: This was corrected when the cabinet in the infant room was locked and the bottle in the toddler remove was stored up high.

It was determined that the caregivers failed to maintain supervision.

During the investigation inspection an infant was observed sleeping on top of a blanket that was placed on top of the fitted sheet. Note: Pictures were taken and the blanket was removed.

It was found that three children transported did not have the medical authorization on file.

It was found that three children transported did not have the emergency telephone numbers for their parents on file.

Children ages 2 months to 4 years old missed an opportunity for outdoor play.

2 infants were seen in a bouncer at 9:24am and remained in the bouncer until 10:04am. This was corrected when the Director asked the caregiver to remove the infants from the bouncer and place them on tummy time.

3 out 7 classrooms were observed lacking toys . Toys were present in the classroom but they were not accessible for the children to use, the shelves were closed together not giving children access to play and explore their thinking skills and having a moment for sensory development.

During the time of inspection caregivers were not following planned activities.

2 infants were seen sleeping in the restrictive device. This was partially corrected when 1 infant was removed by the caregiver.

2 out of 7 classes required children to remain in restrictive devices for more than 15 minutes. Children ages 18 months to 2 years were in high chairs and they were not eating. Infants ages 12 months to 23 months were seen sitting in a high chair unable to move and they were not eating.

Caregiver involved in incident is lacking 5 hours of the required 24 hours.

Caregiver who was providing transportation during the time of the accident does not have the require 2 hours transportation training.

Caregiver that was in a recent vehicle accident did not know the age of the child being transported.

Caregiver who was providing transportation during the time of the accident does not have the required Pediatric First Aid and CPR.

A child was injured and caregivers does not know what happened to the child during the time of the incident.

A van transported children in the morning and the van was lacking a fire extinguisher. This was corrected at inspection when the director placed an fire extinguisher inside the vehicle.

A child was transported and was lacking emergency medical information. This was corrected when the caregiver placed the information in the transportation file.

3 out of 10 file are lacking emergency contact addresses.

Children were sleeping on cots and mats that were not labeled.

During nap time children were sleeping on nap mats that were not distinguished between floor side and sleeping side.

A child was sitting at a table eating lunch and was not strapped in. This was corrected when the child was removed from the table and placed in a chair that did not require safety straps.
