I observe exposed wiring under a computer desk accessible to children in rooms #16 and #19.

Incident form that was used is lacking Location of incident, licensing notification, medical attention information, doctor's information and recommendation, more details about the incident and privacy statement.

It has been found that a child was injury due to the caregiver not being able to keep the children under control.
