Two caregivers do not have first aid and CPR training.

One child's file does not have a copy of their immunization record. One child's file does not have an updated immunization record.

Based on the preponderance of information obtained during the investigation there is sufficient information to prove that a child had ran out of the cafeteria unsupervised and was hiding in a classroom for approximately 10 minutes. Note: Caregivers were actively looking for the child during that 10 minutes.

There is paint chipping in the infant room, pre-k, and toddler room.

Based on information received during the DFPS investigation a caregiver has allowed a 5 month old infant to sleep in a restrictive device.

Based on the preponderance of information obtained during the investigation a staff member admitted to telling a child that they will call their parent and they will get whooping?s/spankings when the child is misbehaving.

Infants had blankets covering the front of their cribs while taking naps. Note: Pictures were taken for the file. Blankets were removed from the sides of the cribs during my visit.

The doors leading into the classrooms have visible dirt on them. Two walls in the school age room are broken. One kitchen toy has visible dirt on it. An outlet cover is broken. Note: Pictures were taken for the file.

The outlet cover was observed broken in the school age classsroom. There was a hole observed in the wall of the pre-k room. Follow up 1/12/22: The door frame has been repaired. The car has been removed from the playground.The ceiling fans have been replaced and no longer wobble. The ant piles have been treated and removed.

The operation continued to have a caregiver present at the operation with an ineligible background check.

Based on the preponderance of information obtained during the investigation there is sufficient information to prove that a caregiver yells at the children as a form of punishment.

Based on the preponderance of information obtained during the investigation there were about 20 children on the playground ranging in age from toddlers to school age children with one staff member.

Based on the preponderance of information obtained during the investigation there is sufficient information to prove a caregiver hit a child in the back with a shoe as a form of punishment.

Based on the preponderance of information obtained during the investigation caregivers were not adequately supervising children during outside play time.

One child's immunization record was not current in their file.

A child was observed on a tablet during nap time.

Two children's files out of six reviewed are missing emergency contact information on their admission forms.

A caregiver was observed at the operation after receiving notification of an ineligibity on 11/10/21.

Milk was observed in the fridge with an expiration date of 11/15/21. Note: Pictures were taken for the file.

The door frame was observed broken exposing a nail. The tires on the car equipment outside was observed to be shredding and exposing wires that are sharp. The ceiling fan in the nap room was observed on and wobbling. Two active ant piles were observed on the playground. Note: Pictures were taken for the file.
