Two out of two caregivers who were observed working in the infant classroom did not have current updated SIDS/Shaken Baby trainings in their files reviewed.

During a walkthrough of the operation, there was observed to be peeling paint on the walls in the nap area that were accessible to the children in care. The peeled paint was also observed to be lying on the floor.

One classroom and the cafeteria was observed during the walkthrough to have multiple outlets uncovered. All outlets were covered before the conclusion of the inspection.

Two out of two vehicles utilized to transport children in care contained fire extinguishers that were not securely mounted in the vehicles.

There was not observed to be a carbon monoxide detector in the operation. The operation's director ordered a carbon monoxide detector during the inspection. It should be delivered on 10/24/25 per the order confirmation.

Diaper changing mats in the infant and toddler rooms were torn, exposing the underlying foam.

The walls in the boys restroom have paint that is peeling from the wall and also has a strong odor.
