The operation's last annual fire inspection was done on 10/3/23.

During the investigation, there was sufficient evidence found to determine that a caregiver used inappropriate discipline when she dragged a child to her, lifted him by one arm, and carried him across the room by one arm.

During the investigation, there was sufficient evidence found to determine that a child was able to get into a storage room that was secured from the inside with a block of wood before the child's caregiver intervened.

The operation did not have a policy informing parents that an animal is present on the premesis.

The playscape, in the playground area, is designed for children 3 years old and older. During the inspection children 18 months to 2 years old were observed on the playscape. The Director placed another caregiver on the playground to assist with providing the children with alternate activities and the children were removed from the playscape.

It was determined the caregiver did not have the appropriate proximity to the children in order to intervene, during the times the children were biting. The ratio in the classroom was 4 to 1. At times, the classroom had 2 caregivers and biting was still occurring frequently.

Based on evidence obtained, it was determined a caregiver will tap children harshly on their shoulder with an open hand, when they do not follow directions given.

Based on the evidence obtained it was determined a Caregiver will yell at the children in the classroom when they do not listen to instruction and loses her patience.

During an investigation, it was determined a child was hit on the thigh by the open hand of a caregiver.

During an investigation, sufficient evidence was found that a child was abused.

Observed children in care transition from outdoor play to nap time without washing hands prior to going to cots. This was corrected by the caregiver taking children to the restroom to wash hands.

Based on the information gathered during the investigation it was determined that the operation did not notify parents allegations that placed a child at risk.

During the inspection the children records were missing the following required documents: Immunizations and health statements

During the inspection the infant bottles were not labeled. This was corrected by the caregiver labeling the bottles.

During the inspection the last fire marshal inspection was completed in July 2021.

During the inspection it was found that the last gas inpsection was completed in March 2020.

During the inspection it was found that the personnel files were not being maintained and the following required documents were missing: date of hire, proof of pre-service exemption, orientation, high school diplomas, proof of trainings within the first 90 days.

Based on the information gathered during the investigation it was found that the operation did not notify licensing within the required timeframe of a child injured at the operation and going to the emergency room for medical care.

The operation did not have documentation that the consumer product safety had been reviewed with the required time frame.

Observed infants sleeping in a room next to the infant playroom with a wall that blocks the view of infants sleeping and the caregiver was observed on the playroom side leaving the infants unspervised.
