Holes in the turf of the playground with green turf present a potential tripping hazard to children and had not been repaired as of the date of the inspection.

The operation did not have any paperwork test data or maintenance instructions available for the unitary surfacing.

Neither play structure had manufacturer's instructions available, making it impossible to assess if the equipment was being used per manufacturer's instructions.

This standard was evaluated as part of an investigation by DFPS and was found to be deficient. The DFPS investigator reviewed a video and noted multiple incidents where a child in care was grabbed by a caregiver.

During the investgation inspection it was found that an infant under 12 months was left sleeping in a restrictive device for over 1 hour.

During the investigation inspection, it was found that a caregiver put a blanket in a crib with an infant under 12 months old.

CCR was not notified within two days after an employee was arrested and charged with a crime. The director stated that she will notify CCR if another similar situation arises.

The operation was aware of an allergy a child had but did not have a food allergy emergency plan for the child.

The operation was made aware through emails and the enrollment form that the child had an allergy to dairy, but still served the child food that he was allergic to.

During the review of children's files, one child did not have a copy of their immunization record available for review, and one child did not have their most current (according to their age) available.

At the time of inspection, no documentation for the required lock down drills were available for review.

One child in care did not have their age required vision/hearing screening on file for review.

Upon review of children's records, three children enrolled into care did not have the required health statement on file for review.

At the time of inspection, a caregiver did not have a notarized affidavit, completed documentation of orientation training being completed, and no qualifcation documentation on file for review.

During the inpsection time, a caregiver was observed being a little rough with the children in care. The caregiver was observed to be pulling the children's arm, slamming down in a child, yanking items away from a child, and placing them down on their mats to sleep a little hard. This was also observed by the director. The caregiver was pulled from the class during the inspection time for the inspector to discuss these observations with her. The caregiver seemed to be more gentle with the child following this interaction.

The operation did not complete the required survey to indicate the number of employees that left the operation during the year 2022.

The vehicle used to transport children does not have a child safety alarm currently installed. The vehicle was purchased and has been in use since August 2022.

The fire extinguishers in the facillity were last serviced in December 2021, according to the service tags.

A fire extinguisher was also not present in the vehicle at the time of inspection.
