During a follow up inspection, four employees still had not completed clock hours in following topics: -Emergency preparedness -Preventing and controlling the spread of communicable diseases, including immunizations. -Administering medication -Preventing and responding to emergencies due to food or an allergic reaction -Understanding building and physical premises safety -Handling, storing, and disposing of hazardous materials

During a follow up inspection, one child's admission form was still missing the physician's information.

The play structure on the Three's and Four's playground is slightly tilted and is a potential safety hazard.

In the Infant B classroom, I observed multiple bottles of cleaning products on a windowsill that was accessible to children.

One out of ten employee files evaluated has online CPR training however, there is no documentation showing in-person training and testing were completed.

In the Toddlers and Infant B classrooms, the diaper changing supplies were in unlocked cabinets that were accessible to children.

One out of ten children's files evaluated is missing the physican information.

I evaluated two vehicles used to transport children. Vehicle 1 had a first aid kit that was missing tweezers, a working thermometer, and antiseptic solution. Vehicle 2 had a first aid kit that was missing a thermometer and tweezers.

A child's medication with an expiration date of March 2024, was stored in the employee break room.

Based on information obtained, there was sufficient evidence to support ongoing biting in the Toddler classroom.

The nap cots were not labeled in the preschool and discovery preschool classrooms.

The hooks for the children's belongings were not labeled in the Infant D classroom.

The walls in the preschool and infant classrooms had chipped paint in multiple locations.

In the Infant D and Two's B classrooms, caregivers who did not have cleared background checks were present at the operation and had access to children in care.

In the Infant A classroom, diaper changing supplies were in an unlocked cabinet that was accessible to children.

The operation's fire inspection expired on 3/22/2023.

Two caregivers had cpr certification that was taken online and did not have documentation showing the written and hands on portion of the CPR certification were completed.

In the preschool classroom, two electrical outlets were not covered. Note: Corrected when the outlets were covered with safety plugs.

The operation has documentation showing a fire drill was last conducted on July 21, 2022.

In the infant A classroom, multiple children's infant feeding instructions were not up to date.

Multiple caregivers completed CPR/first aid certification online which does not adhere to the CPR guidelines as hands on training with a manikin was not conducted.

Two out of ten employee personnel files evaluated were missing a notarized affidavit.

Two out of eight infant cribs were not labeled. Note: Corrected when the caregiver labeled the cribs.

The cots were not labeled in the two's, fours, and preschool classrooms. Note: Corrected when the cots were labeled.

Based on information obtained, there was sufficient evidence to support a child was left on the playground unsupervised.

The operation did not provide a parent with an incident report within 48 hours regarding an incident that occurred with their child.

Based on information recieved there is enough information to suggest that the caregiver did yell at a child in care.

Director does not have certificates for the additional training hours that was completed.

1 out of 10 children files did not have a food allergy plan on file with a severe allergy.

1 out of 10 children files did not have a physicians health statement.

1 caregivers transportation safety training expired on 11/1/2021

1 out of 10 children does not have an emergency contact address on file.

The operation was storing a bottle of children's medication when the child is no longer enrolled at the operation. Note- This was corrected during the inspection when the medication was discarded.

The director did not have current First Aid certification.

One caregiver had 20.5 out of the required 24 hours of annual training. The caregiver was missing training in the required topics of Administering Medication, Emergency Preparedness, and Buiding and Premises Safety.

Two children's medications were not properly labeled with the children's full name. Note- This was corrected during the inspection when the children's medications were properly labeled.

The director did not have current CPR certification.

The toddler classroom, which had one 16-month-old infant present, had reduced ratio during naptime. There was one caregiver to 13 children with a specified age group of 18-23 months of age. Note- This was corrected during the inspection when the 16-month-old infant was moved to the infant classroom.

A toddler caregiver did not know an accurate count of how many children they were responsible for. Note- This was addressed during the inspection when the caregiver recounted the children in care.

Based on the DFPS investigation, there is sufficient evidence to suggest that abuse occurred.

Based on the DFPS investigation, there is sufficient evidence to suggest that inappropriate discipline occurred which caused an injury to a child.
