An incident was not reported to Childcare Regulation during the required timeframe.

Based on the evidence obtained, there is sufficient evidence of medication not being stored properly and out of reach of children.

During a DFPS investigation, it was found that caregivers are teaching children to hit each other in retaliation.

There was one caregiver with a pending background check providing care to children. This was corrected at inspection once the caregiver was removed from the classroom.

A classroom was observed with 15 children to one caregiver, requires 2 caregivers in the room at all times. This was corrected at inspection once the second caregiver returned to the classroom.

The inside of the infant playground equipment contained spider webs in corners, debris of leaves and grass, and brown insects that are unknown to be harmful to children. The grass level around the tree is tall and not maintained.

The current infant feeding instructions were not available for 6 infants that were present.

There were 4 subjects active on the background check people list that were no longer associated with the operation. *This was corrected at inspection when the subjects were inactivated and the list was validated.

One caregiver did not have the accurate count of children in care. The child was moved into the room during naptime but was not signed in. *This was corrected at inspection when the child was signed in.

The operation has not obtained the most recent sanitation inspection. The last inspection was on 10/24/2022.

*There was dust build-up on the ceilings and around the vents in most classrooms. *There was trash, broken tree limbs and broken containers on the playground accessible to children. *There was a large whole broken out of one of the playscapes, large enough to fit a childs head or cause an entanglement or entrapment hazard. *There was trash in one of the buses used to transport children.

The background check people list was not validated every 3 months as required. The last validation was on 7/17/2023. *This was corrected at inspection when the list was updated and validated.

During the walk-thru the toilets used by the school aged children were observed to be dirty with feces in the toilet and one toilet had the toilet seat cover broken off. This was corrected at inspection when the toilets were cleaned and the toilet cover was replacing.

During a DFPS investigation, it was found that a caregiver was disinfecting soft blocks at a sink that faces away from the classroom while aware of the children playing and climbing the bookshelf.

A child was not made to wash their hands after contact with a harmful substance. This was corrected at inspection when a caregiver assisted the child with washing their hands.

A child was in a bathroom stall alone playing with the water inside of the toilet. This was corrected at inspection when the caregiver removed the child from the bathroom stall.

During the inspection, a piece of the play structure was found to be damaged and in need of repair. During the inspection, an electrical panel was found to have wires exposed and accessible to the children. During the inspection, a restrooms used by the children in care was found to have a strong urine odor, soiled debris was on the floor, liquid was observed on a toilet seat and on the restroom floor.

The operation failed the fire inspection conducted 1/4/23 and have not made the corrections yet.
