The director and employee time sheets were not being maintained. The time sheets available for review were from May 2025.

Cigarette smoke odor was present in the transportation vehicle.

3 of 3 personnel records reviewed were missing a Pre-Employment Affidavit for Applicants for Employment (Form 2912). 1 of 3 personnel records reviewed was missing a notarized Affidavit for Applicants for Employment (Form 2985).

Four cans of Mandarin Oranges were expired in the kitchen pantry cabinet. Note: This was CORRECTED during inspection as the expired canned fruit was discarded.

1 of 4 children's records reviewed was missing the child's immunization records.

There was a lighter on top of the halfway wall which was accessible to the children. Corrected at Inspection: Lighter was removed.

A classroom lacked a designated container for the soiled diapers. Three used diapers were observed in the hand washing sink.

There was an empty beehive in the outdoor play area which posed a risk of children getting stung if bees were attracted to it. Note: The empty beehive was removed during the inspection.

Three children on their highchair were not properly secured. Note: Children were strapped during the inspection.

Three children were alone while the caregiver changed another child's diaper in another room.

Two caregivers did not have an Orientation statement. The transportation training for one caregiver was not available for review.

The sign in log was not up to date. There were 2 children present and 6 were signed in. Corrected at Inspection: The director signed out the children who had left for the day.

There was a small area with water stains on the ceiling in the infant room.
