In the outdoor playground area, the area around the swings and the slide structure had mulch measuring less than half of an inch.

One medication for a child in room 5, had an asthma medication that was not in the original container.

Two of the six trash cans obsserved did not have lids making the trash and garbage accessible to the children.

One child was administered an asthma medication twice in Dec. 2023 but the medication at the operation expired in October 2023. Inspector also observed one inhaler spacer not in the original container and it had no name on it.

One out of five staff was missing emergency preparedness training. One out of five staff was missing food allergy training.

One out of five staff did not have their record/file at the operation at the time of inspection. Two out of five staff did not have notarized affidavits on file.

The fire inspection on file was from 8/9/22.

One staff had expired First Aid/CPR Certification on file. The certification expired in January 2023.

Upon arrival to the inspection, staff did not know who the person in charge was in the absence of the director. The employee list was not updated and did not have the current director listed or current employees to show who the person in charge was.

Based on the information gathered through this investigation, a care giver stated a incident report was not done due to her not being trained how to fill out a incident report. The teacher was not present at the time and was out and did not give the parent the incident report till three days after the incident occurred.
