Observed children ages 19 months watching Ms. Rachel on a tablet. I spoke with the caregiver about children under the age of two not having screen time. The tablet was turned off during the inspection.

During this follow up inspection it was observed that there was a person in a classroom with children without a background check on file. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

Observed an infant in a crib with a bottle. The bottle was removed from the crib.

One caregiver did not know how many children that they had. The caregiver stated that they should have 11 children, but there was only 10 children in the room. The caregiver counted and stated that one was moved.

Observed a caregiver using thier phone for Tik Tok. This was corrected when the caregiver put the phone away.

Observed Lysol and disinfectant spray under an unlocked cabinet in a bathroom that is used by the children in care. This was corrected when the cleaning supplies were removed and placed out of the reach of the children.

There was two people at the operation without a cleared background check. One person was at the operation for a few hours to answer phones and the other person has been at the operation a couple of days to wash dishes. An administrative penalty will be imposed in the amount of 100% of the daily maximum per statute, HRC 42.078(a-2).

Observed the last fire inspection conducted on 5/2/22. I spoke to the director about ensuring that a fire inspection is completed each month.

The operations last sanitation inspection was 5/3/22. I spoke to the director about getting a health inspection each year.

During the investigation it was found that a parent was not notified of a minor injury of the child. This was corrected when I spoke to the caregiver about letting the parents know about minor injuries.

During a walk-through of the childcare facility, an infant was observed using infant equipment and the safety straps were not fastened. The infant was immediately removed from the equipment when addressed bringing the facility into compliance.

During a walk-through of the childcare facility, a toddler classroom was out of ratio by one child. A child was removed from that classroom and placed into another classroom, bringing the toddler classroom ratio into compliance.

Three children were observed using equipment that was equipped with safety straps and the safety straps were not fastened. The safety straps were immediately fastened when addressed, bringing the facility into compliance.

After a review of children's records, a child enrolled in care did not have a physician's health statement on file for review. The child was enrolled in care in January of 2020.

After a review of employee files, there were no affidavits for applicants for employment on file for review.

A 16-month-old infant was observed to be asleep in an infant crib with a bottle. The bottle was immediately removed from the crib, bringing the facility into compliance.

After a review of employee files, there was no educational documentation to determine if employees met the educational requirements.
