One out of ten children's records reviewed is missing health statement and hearing / vision screening.

The director was missing annual training on the following topics: (1) Recognizing and preventing abusive head trauma; (2) Understanding early childhood brain development; and (3) Preventing and responding to emergencies due to food or an allergic reaction. Note: The following trainings were taken during the inspsection; recognizing and preventing abusive head trauma, and Preventing and responding to emergencies due to food or an allergic reaction.

Three of Ten children's records reviewed were missing a written statement, from a health-care professional who has examined the child within the past year.

Based on the information gathered during this investigation, proper supervision was not established when a child got hurt while the caregiver caring for the group of children was writing an incident report for another child. This prevented the caregiver from intervening when necessary to ensure the child?s safety.

The annual fire inspection report was not current. A fire inspection was last conducted on 8/20/2021.

A list of a child's food allergies was not posted in the kitchen and in each room where the child spends time. Note: This was CORRECTED during the inspection as a list of the child's food allergies was created and posted in the kitchen and in each room the child spends time in.

A caregiver conducting transportation was missing current annual training in transportation safety. The last time the training was completed was on 7/29/2021.

Soiled diapers were observed in a trash can with no cover in a toddlers restroom. Note: This was CORRECTED during the inspection as the trash can was replaced with one that had a cover.

There was no emergency lighting available in a Pre-k room. Note: This was CORRECTED during the inspection as a battery operated flashlight was placed in the room.

The emergency medical transport and treatment authorization forms were missing in the transportation vehicle binder for 12 children being transported. Note: This was CORRECTED during the inspection as the required forms were obtained and included in the transportation vehicle binders.

Ratios were not met in the infant room when 3 caregivers were caring for 11 infants. The specified age of the children in the group was between 0-11 months. According to the ratio chart the maximum number of children two caregivers may supervise is 10.
