An infant was observed swaddled in a crib.

An infant room had 8 children present with one caregiver. The specified age of a 12 month to 17 month room is five infants to one caregiver. This classroom was over ratio by three children.

A child was observed outside of the classroom walking to the restroom alone.

Five children were observed in bucket seats not buckled in.

The diaper changing pad in the toddler room had a large tear which made the changing pad absorbent.

A caregiver did not wash their hands after assisting children with the toilet.

Operation lacked a current fire inspection.

During the course of the investigation, it was determined that children under the age of two were watching television.

During the course of the investigation inspection children were observed in highchairs (restrictive devices) which were being used as a form of discipline due to the children throwing blocks.

Two caregivers were observed changing diapers. They did not wash the children's hands afterwards.

There was not an extra person in the building during nap time.

RECITE: Two out of four children's records evaluated lacked an updated shot record on file.

The boys bathroom was observed dirty with dead insects and trash on the floor.

Two out of four children's records evaluated lacked an updated shot record on file.

Two out of four personnel files evaluated completed CPR training online.

Based on information gathered, a caregiver has pinched multiple children.

Based on information gathered during the investigation, a caregiver failed to use good judgment when she kicked children's mats during nap time.

Garbage in the boy's bathroom was overflowing from the trash can and onto the floor. Note: This was corrected when the director took the trash out.

The Director failed to utilize tools provided by Child Care Regulation to assist in evaluation of the operation classrooms, required posting, and staff files, resulting in non-compliance at the time of review.

One caregiver was observed caring for twenty-three children. The specified age group was three years old; therefore the caregiver was over child/caregiver ratio by eight children.

The Pre-K children were not given snack until being instructed to do so by licensing, which was after 4:00pm. Note: This was corrected when the children were given snack.

Sink in the Toddler classroom is not working as the water does not come out when faucet is turned to "on".

Prek Classroom had two rugs, green road rug and alphabet rug, having frayed edges posing a tripping hazard.

Directors and teacher did not exercise good judgment by allowing two children to be unsupervised playing with water in the restroom for 8 minutes.

Two children were observed being unsupervised in the restroom playing in the sink with soap and water and paper towels. These two children were unsupervised for 8 minutes and neither teacher acknowledged the children missing.

Sink in the Toddler classroom is not working as the water does not come out when faucet is turned to "on".

Teachers did not account for two children in their classroom as the two children played in the restroom unsupervised for 8 minutes and neither teacher acknowledged the children missing.

Posted activity plan in the Prek Classroom was outdated as the plan posted is for the week on January 18-21, 2022.

Caregiver failed to maintain ratio in the prek classroom for a short period of time. The specified age group for the prek classroom, at this time, is 15 for one caregiver and one caregiver took four children to the restroom leaving the other caregiver with 20 children in one classroom.

It was determined the caregiver did not demonstrate good judgement when she aggressively pushed a child to the floor because the child pushed the caregiver; this was witnessed by another caregiver.

Caregiver did not interact positively with one child's behavior as the child pushed the caregiver and the caregiver pushed the child back aggressively making the child fall to the ground. This was witnessed by another caregiver.

Through information received with this investigation, it was determined that prohibited punishment was used when on caregiver pushed a child to the ground aggressively.

RECITE: A piece of wood was observed to be split apart posing a safety hazard. Screws were also observed to be protruding from two pieces of wood posing a safety hazard.

Director did not have the 30 hours of annual training within her annual training year. Director has completed 21.

One classroom was observed to be out of ratio with one caregiver supervising 17 children. The specified age group for the three year old classroom is 15, the classroom is over by two children.

One personnel record reviewed did not have a copy of their high school diploma, General Equivalency Diploma, on file available for review at the time of inspection.

A piece of wood was observed to be split apart posing a safety hazard.

One personnel record reviewed did not have a copy of her photo identification on file available for review at the time of my inspection.

RECITE: Two children was observed walking out of the classroom to the restroom which is located down the hallway unsupervised.

At the time of inspection, caregiver was observed to be on her cell phone while children were playing in the classroom.
