During the inspection, a 3-month old infant was observed sleeping on a restrictive device. Inspector asked caregiver to move child to a crib and this was completed. Later in the same inspection, the same child was observed sleeping on a bouncer.

During the inspection, it was observed that a caregiver was using high chairs to keep three children seated over 15 minutes after eating as the caregiver mentioned that the children are unable to remain seated during activities.

Last fire drill practiced was on 07/12/2025. The fire drill practiced in August was not documented in the form.

Last fire inspection conducted was on 8/15/2024 and documentation was not provided of why inspection was not available.

In two different restrooms accessible to children, there were cleaning products and shaving creams stored under the sinks, which lacked safety locks.

Four employee records were reviewed and were missing the following records employment begin date, employment affidavit, copy of photo id, and employee statement center policies/personnel policies.

Staff files were reviewed and 3 out 4 were not available for review.

It was observed that the television was turned on showing cartoons in a classroom with 4 infants ranging from 6 months up to 20 months. Please note: Television was turned off during inspection.
