Last gas leak inspection was conducted on 05/10/2023.

A caregiver caring for a group of children was missing 16 hours of pre-service training.

Children in the afterschool room were observed watching a show on the television. The show playing (Lemonade Mouth) was not related to the planned activities. Two children were observed playing roblox games on tablets and a third child was observed playing a fighting game on a tablet.

A caregiver in a odder room was heard yelling at children as a form of discipline.

The activity plans posted in both infant rooms, and both toddler rooms were not current. The activity plan in one infant room had a date for the month of October 2023. The activity plan for the second infant room had a date of May 2023. In the toddler rooms the dates of the activity plans posted were from November 13, 2023 and August 2023. Note: This was CORRECTED during the inspection as the current activity plans were obtained for each room and posted.

The most recent Child Care Regulation inspection form was not posted.

A seat in one of the transportation vehicles was observed with several tearing exposing the foam material inside of the seat. This posed a safety hazard as the foam material was accessible to children.

1 of 6 children's immunization records reviewed was not current.

A 16 month old infant and a 17 momth old infant were grouped together with older children. in The oldest child in the group was 10 years old and the youngest child was 16 months old. The oldest child in the group was more than 18 months older than the youngest child.

A caregiver was caring for 5 infants in the infant room. The specified age of the children in the group was between 0-11 months of age. According to the child/caregiver ratio chart, the maximum number of children a caregiver may supervise is 4.

A soiled diaper was observed in a trash container without a cover. Note: A new trash bag was placed in the container and caregivers were notified to use the diaper genie to dispose of soiled diapers.

A 17 month old infant and a 19 month toddler were observed with a group of children watching a movie during naptime.

Based on the information obtained during the investigation, it was determined that children were not adequately supervised when a caregiver allowed older children to carry younger children while the caregiver was riding on a skateboard in the classroom.

Based on the information obtained during the investigation, it was determined a caregiver used inappropriate discipline on a child when the caregiver yelled at a child and forced a child to get up by grabbing the child from their arms.

Based on the information obtained during the investigation, it was determined that a caregiver did not demonstrate good judgment when the caregiver allowed several children in care push the caregiver around on a skateboard in the classroom.

Three infants were sleeping on a restrictive device (floor bouncers) for an undetermined amount of time.

A caregiver who conducts transportation did not have current training in transportation safety. The certificate of training available for review had a date of completion 5/16/2020.

Three cribs were not labeled with the assigned infant's name. Note: This was CORRECTED during the inspection as the cribs were labeled with the infant's name.

A mop bucket containing water was observed in the afterschool classroom area accessible to children. Note: This was CORRECTED during the inspection as the bucket was removed from the room area and placed outside out of children's reach.

The immunization record reviewed for 1 of 6 children's files were not current.

The number of infants in care exceeded the infant room's activity space. There were a total of 6 infants in care with two caregivers during the inspection. Based on the space measurements for that infant room the maximum number of children is 5.

The operational policies reviewed were missing the following information: Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan); your policies must include: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

An uncovered container with food was observed in the refrigerator. Note: This was CORRECTED during the inspection as the food in the container was discarded.
