Changing pads in two classrooms had rips and tears. Operation came into compliance at time of inspection by removing damaged pads from classrooms.

three out of ten children's up to date vaccination records were not available for review.

Infant bottles were observed not being sanitized after each use.

During the course of the investigation, it was determined that staff were restraining a child to keep them in time out.

A caregiver was observed pulling her ringing cell phone out of her pocket and looking to see who was calling. The caregiver was working in in the infant room and counted in the child caregiver ratio.

Four written feeding plans were not signed by the parent.

One enrollment form did not have the Emergency Medical Treatment section completed. The violation was corrected at the time of inspection. The parent came to the center to complete the admission form.

The grass on the infant/toddler playground and grass along the fence on pre school playground needs to be trimmed. The discarded toys in the wagon outside the gate on the North side of the building need to be discarded or properly sanitzied before use. The wagon is holding waer.

At the start of the inspection, there was one caregiver present in the building with 14 children ranging in age of 7 months to four years of age.

At the time of the inspection there was on caregiver supervising two groups of chldiren. The caregiver was standing in the classroom with 12 children and was also looking through a door to supervise the two infants who were sleeping in the infant room. The was no caregiver physically present in the infant room.

There was not sufficient staffing in the building for two classrooms with children present in them during the naptime. There was only one caregiver present.

At the time of entry into the programs mail buidling through the operations front door, there was not adequate lighting see arrangment of the buildings contents or the 12 sleeping children in the two year old classroom. The operation is an open concept building.

Duirng naptime the children were served cherry Kool-Aid.

The operation does not have a current fire inspection.
