An infant was not given a bottle during the timeframe that was noted on the infant feeding instructions. The infant feeding instructions stated that the infant needs to be fed every 2-3 hours and the infant was given a bottle after 4 hours.

It was found that an incident report was not completed for an infant that was given the wrong breastmilk.

During an investigation it was found that a bottle was not labeled with the infants first name and last initial. There was also a bottle not labeled during the inspection today. This was corrected when the bottle was labled.

Observed a plastic slide that was missing part of the roofing.

Observed one caregiver state that they had 10 children but there were 11 children in the classroom. Observed a second classroom state that they had 17 children but they had 16 children in the classroom.

Observed a diaper changing mat that was ripped in one of the classrooms.

Observed a caregivers purse in the bathroom in the reach of children with two bottles of medication in the purse. The bottles of medication were at the top of the purse and could be seen just by walking by the purse. This was corrected whent he purse was put out of the reach of the children in care.

Observed one caregiver who has worked at the operation for over a year and only has 16 hours of training for the year.

Children should wash hands prior to eatting any meal served at the operation to prevent the spread of germs.

During an investigation it was found that caregivers sometimes use gloves instead of washing their hands to serve food.

A child was observed on video footage being smacked on the leg by a caregiver. A child was also observed on video being picked up roughly and placed down into a chair in an aggressive manner. A child was observed on video being placed on the floor, hitting their back against the wall in an aggressive manner.

Caregiver appeared by video to place a child down at a chair using more than normal force. Caregiver appeared to be flustered and not have control of their actions. Caregiver was terminated by operation.

During the investigation, there was sufficient evidence found to determine that a child was not being adequately supervised when the child was bitten multiple times before a teacher noticed and intervened.

During the investigation, there was sufficient evidence found to determine that children under two years old were watching media on an iPad.

During the investigation, there was sufficient evidence found to determine that the operation had over-the-counter medication that was being used for multiple children.

The director had not reviewed the children's product recall list. This was corrected during the inspection when the director was shown how to subscribe to the recall emails.

The fire extinguisher in one of the vehicles needed to be serviced as the needle was in the red.

In one of the children's bathrooms, there was a garbage can with dirty diapers that did not have a lid on it. This was corrected during the inspection when a lid was placed on the garbage can.

During the DFPS investigation, there was sufficient evidence to show that a caregiver spanked a child in care.

A caregiver was observed caring for children, and she did not have a cleared background check. The caregiver left the operation during the inspection.

During a DFPS investigation, it was found a child in care was neglected.

During a DFPS investigation, it was found the electronic safety alarm was disabled and not being used when transporting children in care.

During a DFPS investigation, it was found a child was left unsupervised for approximately two hours on a transportation vehicle.

During an investigation it was observed through video footage that caregivers were sitting down on the playground not properly supervising the children when a child in care was injured.

An infant was sleeping in a crib with a burp cloth in the crib near the infants face. The burp cloth was removed from the crib. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

A caregiver did not use good judgment when they picked a child up by the childs leg and one arm to lay the child on their mat.

During an investigation it was found that a caregiver was aware of allegations of abuse and did not report the incidents.

A caregiver was changing a young child in the bathroom and had up to 4 more children behind the caregiver. The caregiver could not see the other children in the bathroom and a child was able to climb on a tall unanchored shelf causing it to tip over and injure a child in care.

The operation added a tall shelf to a classroom bathroom but did not anchor it to the wall. A child was able to climb on the shelf which caused the shelf to fall and injure a child in care. This was corrected when the shelf was removed from the bathroom.

The operation had completed a boo boo report, but had not completed the licensing incident report for a child who received an injury while in care which required medical attention. This was corrected when the operation completed the correct incident report.

Observed 5 infants sitting in infant high chairs without safety straps being used. There were no safety straps on the chairs due to them being washed. This was corrected when the straps were added to the chairs.

Observed a caregiver snatch a toy from a child, grab a childs arm and then picked the child up by the childs hand as the child's feet left the floor and took the child to another area in the classroom.

Observed a 2 month old infant sitting in a boncy seat while the caregiver sat beside the infant holding the infants bottle. This was corrected when the caregiver was told that infants under 6 months old must be held to be fed and the infant was picked up.

Observed two caregivers with access to the children who did not have a cleared background check at this operation. This was corrected when the caregivers were asked to leave the operation and a background check was put it.

Observed dirty diapers on the floor of the bathroom. This was corrected when the diapers where thrown in the trash,

Observed 14 staff without cleared background checks. This is being corrected by scheduling FBI fingerprint appointments for the staff. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.
