Based on the information gathered throughout the investigation, it was concluded that the caregiver grabbed the child by the leg during naptime, which resulted in visible red marks on the child?s thigh.

Based on the information gathered throughout the investigation, it was concluded that the caregivers did not implement appropriate redirection or use positive guidance methods when addressing the child?s behavior during naptime.

A hole was noted in the hallway wall. Additionally, the turf in the playground area, specifically at the base of the slide, showed signs of damage. Photographs were captured for documentation.

Based on information gathered during the investigation, it was determined that the caregivers involved in the incident did not intervene to remove the victim from a dangerous situation, speak up to prevent harm, or provide additional help for the victim.

Two children with food allergies did not have a complete plan on file that was signed by the parent and/or doctor.

The emergency medical transport and treatment authorization forms for each child being transported was missing information.

During the inspection, it was noted that in the 18-23 months classroom, one child did not wash their hands after a diaper change. This issue was promptly addressed during the inspection when the caregiver was instructed to wash the child's hands.

The operation allowed two caregivers to be present at the operation before receiving notice from the CBCU regarding their eligibility. The operation immediately removed the caregivers from the operation.

A child was left unsupervised on the playground for 10 minutes.

Five children ages 18 to 23 months were observed watching a video on a tablet in the classroom during the inspection. Note: it was corrected during the inspection when the caregiver turned the tablet off.

One caregiver's records out of ten reviewed did not have the following training: Preventing and responding to emergencies due to food or an allergic reaction and Handling, storing, and disposing of hazardous materials

The documentation for the unitary surfacing material (turf) was not available to review during the inspection.

A caregiver has an expired pediatric CPR/FIRST AID certification.

A 5 year old child was left unsupervised in the classroom for about 6 minutes when the class transitioned to an outdoor activity and the child was unharmed.

During inspection the designee who is also a driver did not have the required First-Aid and CPR training.

The operation did not report a positive COVID-19 case to Child Care Regulation within the required timeframe.

A caregiver did not demonstrate self control when interacting with toddlers in care that were not sleeping during naptime. The caregiver repeatedly placed one of the children on their stomach in a sleep position and then moved the rolling cribs back and forth in a firm manner.
