An infant play toy was observed with dirt/dust accumulated on it. This was corrected when the toy was cleaned. A few toy bins in a room were observed to be dirty. This was corrected when the bins were cleaned.

Two infant bottles were observed with only the first name. This was corrected when the staff added the last initial.

Based on the video, a child was acting out and as the child was being spoken to by the teacher about this behavior, the child was seen hitting the teacher several times on the back and arm. The caregiver continued to pick up papers off the floor that the child throwing on the floor. The child continued to hit the teacher. When the caregiver turned around towards the child, she tried to grab for the child?s arm that was hitting her but missed and that is when the child hit the teacher in the face, the teacher reacted by swinging her hand that was holding the papers toward the child and the papers hit the child in the face causing a slight red mark on her cheek from the papers only. There was no bruise or bump. NOTE: The operation was proactive by immediately terminating this employee.

Two purses were found in rooms accessible to children. This was corrected when the purses were put up out of children's reach.

An infant saucer was observed in the room to be dirty with no child in it. A bookshelf in a room was observed to have dirt inside of it. There were several plastic containers in two rooms that were cracked and sharp to the touch for children. These were corrected during the inspection.

IDuring the course of a DFPS investigation it was determined that a caregiver in the 3 year old classroom had direct access to children without receiving notification of eligibility to be present from the background check unit. The caregiver was terminated on 9/13/2021.

Based on the information gathered and video observed during this investigation, a caregiver was seen picking up a few children roughly and incorrectly while placing them in a different area on the floor. There were no marks ever seen or reported by anyone. An investigation was immediately conducted by the center. The caregiver was immediately terminated.

Upon review of the video, the caregiver was observed cleaning the room (stacking chairs and putting toys and nap mats away) for over 5 minutes while the children were running around the room and climbing inside the book nook.

A caregiver was observed yanking a child out of a book nook by their forearm and dragging them a short distance away. The caregiver then set the child down roughly on their bottom and then grabbed the child by their face while scolding them verbally. The operation immediately terminated the caregiver.

The caregiver was heard redirecting children from across the room, using negative terms such as "No, stop that, don't do that." The caregiver also threatened children with no outside time. The caregiver then grabbed a child by their cheek and told the child, "you don't say no to me."

The 4-year-old room gave the wrong count during the inspection. The teacher gave a count of 22 children instead of 21. The teacher said she forgot to remove a child from the iPad. This was Corrected when the children were recounted by another staff in the room.

A child in the 18-24 year old room was observed on a sleeping mat blocking the exit back door. This was CORRECTED when the child was moved.

The staff did not use good judgment when they did not conduct a head count, name to face or a sweep of the room to ensure all children were accounted for as they were transitioning from leaving the room to go outside. This resulted in a child being left in the bathroom and coming out to an empty classroom for approximately 2 minutes.

9 out of 10 employee files were observed to be missing signed and dated orientation statements.

6 bottles in 2 rooms were observed to be missing labels of the infant's names. Note: This was corrected at inspection when caregivers labeled the bottles with marker and tape.

5 out of 10 employee files were observed to be missing proof of education documentation.

3 out of 10 employee files were oberved to be missing the Notarized Affidavit.
