During the investigation inspection, one classroom stated that 14 children were present, however a 15 children were present in the classroom.

Based on the information obtained through the DFPS Investigation, the operation was notified of the incident and they did not make a report to licensing.

Based on the information obtained throughout the investigation, there is sufficient evidence to show that a caregiver used inappropriate language towards another caregiver in the presence of children.

During the inspection, two caregivers in two separate classes were heard to be yelling at children to get the children to listen to them.

Based on the information obtained, there is sufficient evidence to show that caregivers have spanked children as a form of discipline.

There is evidence to show that boppy pillows were utilized in lieu of holding 4 and 5 month old infants when giving a bottle.

Based information obtained through the DFPS investigation, there is evidence to show that a caregiver was grabbing and pulling a child to get them to sit still in one place.

Based on the information received from the DFPS Investigation, there is evidence to show that a child spent approximately 55 consecutive minutes in a bouncer.

The operation did not create a Incident Report for a child running out of the room, and did not share it with the parent.

There is enough evidence to show that a child walked out of the room and outside. The caregiver followed, however he was alone for a short amount of time.

The operation did not report to licensing that a child walked out of the room unattended and went outside.

Caregivers in three classrooms did not know how many children they were caring for.

The 12-24 month old classroom was out of ratio. The room had 7 children to one caregiver. The correct ratio for the specified age group is 5 children to one caregiver. NOTE: A caregiver was added to the ratio during the inspection.

PG movies have been shown in the mornings to children 2 and older.

One bus was missing scissors in the first aid kit. The second bus was missing scissors and a thermometer. NOTE: This was corrected at inspection when the items were put into the first aid kits.

There was an exposed bolt on the play structure on the toddler playground.

I observed that the first aid kit in the bus did not include tweezers or thermometer.

I observed an food allergy emergency plan for a child that had not been signed by the parent. NOTE: This was corrected during the inspection.

I evaluated 10 children's records. I observed the following information missing from the files: 1. One child was missing the preschool health statement. 2. One child was missing the address for the emergency contact person.

In suite 350 there was a bottle of disinfectant that was on a low shelf accessible to the children. Note: A picture was taken. Note: The bottle of disinfectant was removed.
