The chains on the swing set in the school-age playground were wrapped around the top bar, rather than being properly attached using the manufacturer-installed swing attachments.

The loose fill on the playground areas did not have the appropriate depth of loose fill.

The operation did not have emergency medical forms in the transportatoion binder.

All staff files evaluated were missing the date of employement. Three out of six staff files were misssing education qualifications. Two out of six staff files were missing their employee affidavit. Four out six staff files were missing the required training hours. Three out of six staff files were missing their photo IDs. Three out of six staff files were missing CPR/First Aid.

The loose fill on the playground areas were not at the appropriate depth of 9 inches.

An individualed was observed caring for children enrolled at the operation, but did not have a background check.

Multiple play structures were observed to have nails protruding from the equpitment and on the boarder surronding the playground. Some play equiptmet was also observed in need of repair.

The operational policy was missing a section on Infant Safe Sleep.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

The director was observed to not have the following required trainings which include: (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety (6) Handling, storing, and disposing of hazardous materials

Two staff members were observed to not have a current CPR/First Aid training.
