Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support the allegation that a caregiver did not intervene prior to a child running out of the broken fence. The caregiver was aware that the child was a known runner, and did not prevent the child from entering the area.

Based on the evidence obtained during the DFPS investigation, the wooden fence had a hole in it in which a child was able to get out the fence.

Based on the evidence obtained during the DFPS investigation, a child was placed in a highchair outside of mealtimes on a frequent basis in an attempt keep the child from running away. Additionally, a caregiver did not display good judgment when they failed to use name-to-face to verify that all children were accounted for while on the playground. As a result, a child was able to leave the playground unattended.

This standard was found deficient as part of a DFPS Investigation. Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support the allegation of neglectful supervision.

Based on the information obtained during the investigation, the operation did not supervise all the children in care which resulted in children being bitten or scratched by one child repeatedly.

There is no documentation that a fire drill was conducted in October.

The loose fill has not been maintained. Several inches of loose fill are missing. Grass is growing in parts of the loose fill. Photos were taken.
