During the investigation, it was found there was no timesheet documentation for the director.

In the Preschool 3 classroom, multiple bottles of chemicals were stored on shelf within children's reach due to the presence of a chair underneath.

In the preschool 2 classroom, I observed a wall with multiple holes that were accessible to children.

A caregiver in the Preschool 3 classroom was heard yelling at a five year old child multiple times throughout the inspection.

Three children with documented food allergies have food allergy emergency plans on file however, they are not signed by a physician.

During a follow up inspection, an 11 month old child who is not on table food does not have infant feeding instructions on file for review.

In the infant classroom, I observed an infant sleeping in a crib that was labeled with another child's name.

During a walk through of the operation, children were napping on the stage area however, there was no room to walk between their mats.

In the infant classroom, I observed a child's spoon in the handwashing sink.

There were no infant feeding instructions on file for a seven month old child who is not on table food. Note: This was corrected when the infants feeding instructions were obtained from the parent.

I observed a five year old child using a tablet during nap time.

I observed multiple cords hanging from a television that was connected to a power strip, posing a strangulation hazard.

In the Toddler classroom, the hooks for the children's belongings were not labeled. Note: This was brought into compliance when the caregiver labeled the hooks by name.
