During the insepction, the operational handbook was viewed and was missing statement related to safe sleep.

During the inspection, there were 5 out of 10 staff files viewed that did not have their 24 hours of training for the 2024-2025 year. 1 staff had 10 hours of annual training,1 staff had 15 hours of annual training, 2 staff did not have any annual training hours 1 staff had 19 hours of annual training

During the inspection, the operational handbook was viewed and was missing statement related to supporting services for children with special care needs.

A climbing structure on the toddler playground does not have loose fill under or around the use zones of the equipment. A blue mettle slide on the toddler playground does not have loose fill under or around the use zones. Note: Loose fill has been purchased and is on site waiting to be added to use zone areas. However, there is not sufficient loose fill to cover the required needs on the toddler playground and refill the structure on the older children's playground.

Three children files out of six reviewed did not have addresses for their emergency contacts.

The children files reviewed did not have information on whether the child can swim unassisted or not.

Mulch on the school age playground did not meet the proper depth.

The fire drills did not contain the time of the drill and length of time from July to October.

Insufficient space was observed between the nap mats in the preschool classroom. SN: Corrected during inspection.

Climbing equipment on the twos playground was observed to have cracks and sharp edges.

The operational policy did not contain a promotion of physical activity section.

The fire drill for last month was not documented.

During follow up inspection, child safety alarm did not come on after exiting the van.

During follow up inpsection, staff did not have a valid training in Pediatric CPR.

During investigation inspection, 1 out of the 2 classrooms observed were watching a show on the ipad. Children were under the age of two years old.

The director/permit holder did not notify Child Care Regulation that the operation was ready to begin providing transportation services.

During investigation inspection, staff did not have daily report sheets avaliable for review.

During investigation inspection, child safety alarm was not working on van used to transport children. Noted: Owner had mechanic come to operation to fix child safety alarm. Once fixed owner brought me outside to hear safety alarm.

In 2 out of the 3 classrooms observed staff did not know the ages or how many children they were responsible for.

During inpection, it was reported by director that she was no longer the director during the time of my visit on 4/26/21. Licensing was not notified of this change until 5/12/21. Noted: Director has since been re-hired.

During inspection, parents had not received form 2970 prior to my visit. Operation had til 5/1/21 to notify parents of the violation of minimum standard 746.2426. Noted: Owner/Director printed and is sending form 2790 to parents.

During inspection, one bottle was observed missing a child's name. Noted: Bottle was labeled by caregiver.

Based on external documentation, it was observed that two infants were left unsupervised.

Two infants were observed to be sleeping in restrictive devices. Noted: Pictures were taken, and children were removed and placed on their backs in their cribs by caregiver.

Staff hired on 4/16/2021 did not have 8 hours of Pre-Service traing before being counted into ratio. Noted: Pictures were taken.

It was observed that staff hired on 4/16/2021 was forcing a child to sleep on cot for naptime. Staff was terminated on 4/22/2021. Noted: Video was taken.
