There was no additional employee in the building during naptime. A caregiver stepped out of the classroom to open the main door leaving the children unattended.

Recite: One child was observed not secured in the chair that was equipped with a safety strap. The seats used by the children now have safety straps.

A bouncing play equipment was observed with less than the needed 6 inches in depth of surfacing.

CPR and 1st aid trainings were not available for review for one caregiver present during inspection. Caregiver was hired over 90 days ago from today.

A play yard in use by an infant was labeled with another child's name. NOTE: This was corrected during inspection. An extra play yard was added to the infant classroom for each infant to have their nap equipment.

Personnel Records for two caregivers were not available for review during inspection. Pre-service documentation for a caregiver and CPR training documentation for another caregiver was not available for review during inspection.

Immunization records for one child were not available for review during inspection. Updated Immunization records for another child were not available for review. NOTE: This was corrected during inspection. Children's parents provided copies of immunization records for both children.

A play yard in use by an infant was labeled with another child's name. Caregivers reported the other child also used this play yard today. NOTE: This was corrected during inspection. Infant was removed from play yard.

Personnel Records for 3 staff members were incomplete. One infant was observed sleeping in a restrictive device: a baby bouncer. Child was not moved as soon as possible to their assigned crib. NOTE: This was corrected during inspection. Child was removed from restrictive device by caregiver.

Two classrooms were deficient in Child/Caregiver ratio as observed during inspection. Classroom #1 had 2 caregivers with 11 infants with a specified age group of 7 months old. Classroom #2 had 1 caregiver with 9 children with a specified age group of 13 months old. NOTE: This was corrected during inspection. One child was picked up by parent from Classroom #1. A second caregiver arrived to Classroom #2.

Two electrical outlets were observed without a cover in the infant/toddler classroom. Outlets were within children's reach. NOTE: This was corrected during inspection. Covers were added to the outlets.

Documentation of February 2022 fire drill was not available for review.

Authorization to administer medication was not available for review for two children taking medication at the operation.

4 children between 11 to 18 months old were observed sitted in high chairs facing a TV in a common area classroom. The two caregivers providing care of children stated children were watching an educational video.

There were 7 infants with a Specified Age Group of 12-17 months old under the care of one caregiver. Child/Caregiver ratio was deficient by 2 children. NOTE: This was corrected during inspection as two children were moved to another infant classroom.

There were 9 infants with a Specified Age Group of 12-17 months old under the care of one caregiver. Child/Caregiver ratio was deficient by 4 children. NOTE: This was corrected during inspection as Director stepped into the classroom until 4 of the children were picked up by parents..

There were two infants asleep in their carseats when licensing inspector walked into the classroom. (NOTE: The infants were immediately removed and placed in their cribs.)

There was 1 caregiver with 7 infants. Of those infants 5 were under 11 months old. (NOTE: The 2nd caregiver arrived and was late due to car trouble)

Caregivers did not wash their hands after diapering children.

There were 11 infants with 2 caregivers. Ages of the infants were 3,4,4,5,4,5,6,7,10,11 and 12 months. (NOTE: Corrected during inspection as another caregiver was called in and 3 of the older children were placed in another room with the aregiver.)

There were a couple of children whos hands were not washed after being diaper changed.
