The operation did not report a child's fractured thumb she sustained while at the center after the child's parent notified them of the injury.

The benadryl for a child in care with a known food allergy and food allergy plan had expired benadryl medication to accompany the plan. The medication expired as of May 2023.

There were several ceiling tiles at the operation observed to have brown staining from possible water residue. The PreK classroom had multiple ceiling tiles/ lighting that appeared to be slowly detaching from the ceiling. The bathroom in the PreK classroom had chipped paint along floor trimming and bathroom cabinet, The playground fence had multiple loose boards or missing boards.

The owner and director were unable to provide any documentation of when the vehicles used to transport children were purchased to determine whether a child safety alarm is required. The operation has a total of 6 vans used to transport and only 2 vans have child safety alarms.

The walls have chipped paint through out the building and wall paper is torn. Pictures were taken.

In January and March the fire drills were not completed in 3 minutes or less.

During a DFPS investigation it observed an infant being swaddle in the crib.

During a DFPS investigation, it was determined that safety straps were not fastened at the infant feeding table.

Based on the information obtained during the investigation, positive methods of discipline and guidance were not being practiced as children were made to do wall sits as a form of discipline.
