During a follow up inspection, the overlapping structures were removed on the school age playground. However, the use zone of the swing and play structure on the Toddler playground were still overlapping.

In the Schoolers classroom, the children were watching "Spongebob" during nap time.

The Infant 2 Classroom does not have an adult sized chair in the room.

On the Toddler playground, multiple swings were observed with rusted and broken seats.

The operation's emergency preparedness plan is missing the following: -How children 24 months and younger or with limited mobility will be relocated from the operation. -An evacuation relocation diagram. -How children will be accounted for at the alternate shelter.

The loose fill material on the school age playground and Toddler playground were not at the proper depth.

In the Infant 1 and Toddler classrooms, fans were mounted on the walls however, the cords were hanging and were accessible children.

The operation's emergency preparedness plan does not state how children will be reunified with their parents.

The door of the fence on the school age playground was observed to have a gap and was accessible to children. In the Two year old classroom, I observed chipped walls and paint. In the Schoolers classroom, I observed chipped paint on the walls and door.

On the school age playground, the use zone of two stationary play structures was overlapping. On the toddler playground, the use zone of the swings and a play structure were overlapping.

The first aid kit on the operation's van was missing tweezers.
