In the one of the after school classrooms the lighting during nap time was not adequate for supervision of the children flashlight was needed to walk around the classroom and supervise the children. NOTE: This was corrected at inspection when caregiver brought a lamp into the classroom that provided adequate lighting to supervise the children.

Outdoor activities are not being incorporated into the infant-age children daily schedule. No written activity plan was available for children in the older school-age classroom.

Two out of the thirty children present were signed -in.

Feeding instructions for two infant-age children not yet eating table food were last certified by parent on 04/2021.

Bottles for infant-age children were not labeled with the children's first name nd last initial.

One out of four staff files reviewed is missing copy of high school diploma/GED or equivalent.

All lights in the prek room were off and windows were covered; children could not be seen. This was corrected at inspection when provider remove the blanket covering one of the windows.

A 8mo old child was observe asleep in a swing.
