Two out of ten children's files reviewed did not have updated immunization record for review.

A child's file reviewed was missing food allergy emergency plan signed by parent and health care professional.

An infant's bottle was not labeled with infant's first name and initial of last name. Note: This was corrected at inspection when director labeled the bottle with infant's first name and initial of last name.

Attendance records or time sheet for director not available at the operation during follow up inspection.

Based on the information gathered, the minimum requirement of 75 percent of director present during operating hours each week or a minimum of 30 hours per week was not met.

One out of the ten staff files did not have current CPR training.

A caregiver held a child firmly by the arm while walking to time out.

The operation did not report the number of employees who ceased employment in 2023 by the required date 1.15.24.

No documentation was provided for the testing of the smoke detector and carbon monoxide.

The fire inspection was last conducted on 4.27.22.

Two trashcans in the PreK3 and PreK4 classroom were observed with food and with no covered lid. NOTE: Corrected at inspection.

Two of the ten caregivers didn't have current training in; CCDBG training (6 hours) which includes Recognizing and preventing shaken baby syndrome and abusive head trauma; Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and understanding early childhood brain development.

No current CCDBG training (6 hours) to include the following; Administering medication, food or an allergic reaction, Handling, storing, and disposing of hazardous materials. In adittion to Safety sleep / shaken baby.

Four of the fire extinguishers were observed to be last serviced in September 2021. In addition to one fire extinguishers in October 2020.

In both of the van first aid kits, no tweezers were missing. NOTE: Corrected at inspection.

One infant feeding schedule was out of date at the tie of inspection.

There was one expired medication at the time of inspection.

At the time of inspection there was not a bottle of bleach water in the infant room for the caregiver to use during diaper changes. This was corrected when the bottle of bleach water was placed in the infant room.

Based on the information gathered at least one instance a communicable illness was not report within 48 hours of the diagnosis.

Based on the observations made and the information gathered, there is not enough toys and activities in the two year old classroom and the school age classroom.
