Weekly activity plans for the six classes were not available to review. This was corrected when updated activity plans were posted by the end of the monitoring inspection.

The operation did not have a sanitation inspection completed in the last 12 months.

One caregivers did not have current Pediatric CPR Training that was taken in person.

Six caregivers did not have annual Handling, storing, and disposing of hazardous materials training

One caregiver did not know how many children she had in care and was responsible for.

One child was left alone in a classroom.

1 staff member did not have their affidavit available in their record.

Two staff members, the cook and administrative assistant, did not have in person CPR training.

This standard was reviewed and found to be deficient due to multiple children stating that their teacher has grabbed their arm at least once. One child stated it hurt when the teacher grabbed his arm.

During an investigation it was found that an incident report completed on 8/5/2022 was not shared with a parent until 8/19/2022.

On 8/5/2022 a 3 year old child was unsupervised in a classroom for approximately 1 1/2 minutes while one group transitioned outside and a second group transitioned into the classroom.

During an investigation it was found that the operation did not report a situation that place a child at risk to Child Care Regulation.

During an investigation, it was found that a child's parent were not notified until 8/19/22 of a situation that placed their child at risk on 8/5/2022.

1 of 6 files reviewed did not have proof of transportation training. 2 of 6 files reviewed did not have proof or SIDS/Shaken Baby training. 3 of 6 files reviewed did not have proof of CPR/FA training. 4 of 6 files reviewed did not have proof of food allergy and hazardous materials training.

Out of 3 caregivers in the infant classroom, no one had current first aid training. Another caregiver with current first aid training was put into the classroom.

A child was given food listed on their food allergy plan two times in one day.

An 18 month old child was unsupervised in a classroom for approximately 7 minutes.

The operation was notified on 11/29/21 at 8:30PM of a doctor's visit from an incident on 11/22/22. The operation needed to notify CCR by the end of the day on 12/1/22 of the medical treatment obtained in order to meet he notification requirement.
