Out of 4 caregiver files reviewed, 2 caregiver were missing first aid training.

The annual sanitation inspection was over due and last completed on 2/9/24.

The last calendar year only had 2 of the 4 required lock down drills and 2 of the 4 required severe weather drills. This was partially corrected at inspection when the operation completed a lock down drill and a severe weather drill.

There were three children with food allergy plans that had doctor signatures but not parent signatures. Two of the children did not have the Benadryl allergy medication as required per the food allergy plan. Without this medication, the plan could not be implemented.

The operation had Benadryl that expired in 2/2025, Neosporin that expired in 9/2024, and Benadryl cream that expired in 7/2024.

The first aid kid in one of three buses had expired antiseptic and was missing a thermometer. The first aid kit in a second bus was missing antiseptic, a thermometer, and tweezers. This was corrected at inspection when the person in charge removed the expired items and added the missing items.

It was found during an investigation that a child was left alone outside in a gated area by an external door for up to 5 minutes.

It was found during an investigation that parents were not notified until to the next day that their child was involved in a situation that placed, or may have placed, the child at risk.

It was found during the investigation a caregiver pulled a child's ear as a form of punishment.

One child did not have a current immunization records.

A caregiver was observed diapering an infant and did not wash the infant's hands after diapering. This was corrected at inspection when the caregiver washed the infant's hands when addressed.

Based on information obtained during an investigation, it was determined that a child was left unsupervised in a classroom. The risk of harm to children has been minimized due to the caregiver no longer working at the childcare facility. The deficiency was corrected at inspection.
