A child in care had their arm pulled by a caregiver, causing injury to the child.

During the inspection, it was discovered that the operation had not conducted their annual fire inspection.

This standard was tasked over for the investigation and found to be deficient. A child was found unsupervised outdoors on the playground while in care at the operation. The center has implemented more thorough name to face checks that have to verified throughout school day by management. Additional supervision and successful transitions trainings have been taken. A 90 day training period has also been put in place.

The person in charge in the director's absence, did not comply with standards by not reporting a situation that left a child with an injury.

Through interviews, it was determined that the operation became aware of a situation that placed a child at risk for injury on 11/30, but did not report until 12/5.

At the time of inspection, the operation' s last documented health inspection was conducted on 5/2/22. No documentation of the most recent health inspection was available for review.

At the time of inspection, the center director did not have the full 30 hours of training accounted for during the previous full year.

At the time of inspection, one caregiver did not have 24 hours of training to accounted for during the previous full year.

This standard was tasked over for this investigation and found to be deficient. A child was left unattended in his classroom while the other classmates and caregivers present went outside to the playground area. Additional training in supervision and classroom management has been assigned to the caregivers present following this incident. The class was observed during the inspection and proper supervision techniques were in place.

One child did not have their most current immunization record, based on their age, available for review at the time of inspection.

The operation's last fire inspection by a fire marshal was conducted on 11/26/2019. Only the fire system inspections have been conducted following this date.

Based on interviews and a video footage, the operation was found not to be in compliance with a standard evaluated for a standard investigation. A caregiver used prohibited punishment to disciplined a child.
