One out of ten staff files reviewed has an expired first aid / cpr training.

Graham crackers and water were served during PM snack. Snack is missing a food group.

Ten staff files were reviewed. Three staff files with foreign education are missing translation / equivalence. Staff training files for six staff members are missing certificates. One file is missing start date and orientation statement.

Two caregivers were missing annual training on the following topics: (1) Administering medication, (2) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (3) Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

A caregiver did not have documentation showing how the caregiver meets the minimum education qualifications.

Two caregivers with education received outside of the United States did not have the education documentation translated in English.

A caregiver was missing 10 hours of annual training for training year 3/19/2022 -3/18/2023. A second caregiver was missing 3 hours of annual training for training year 9/15/2022 - 9/14/2023.

The operational policies reviewed were missing the following information: (1). Hearing and vision screening requirements (2). (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (F) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play; and (G) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play. (3). The Procedures for parents to review a copy of the child-care center's most recent Licensing inspection report and how the parent may access the minimum standards online; (4). ) Instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website; (5). provisions to provide a comfortable place with an adult sized seat in your center or within a classroom that enables a mother to breastfeed her child. In addition, your policies must inform parents that they have the right to breastfeed or provide breast milk for their child while in care; and (6). Under the Texas Penal Code any area within 1,000 feet of a child -care center is a gang free zone, where criminal offenses related to organized criminal activity are subject to a harsher penalty.

Animal feces was observed in the children's outdoor playground. Note: This was CORRECTED at inspection as the animal feces was removed from the playground.

A caregiver with was missing annual training for training year 3/19/2021 - 3/18/2022 on the following topics: (1) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (2) Handling, storing, and disposing of hazardous materials A second caregiver was missing 24 clock hours of annual training for training year 10/23/2021 - 10/22/2022.

A caregiver's training record was not available for review during the inspection.

Two caregivers did not wash their hands after wiping a child's nose in both toddler rooms. Note: This was addressed with the staff during the inspection and staff proceeded to wash their hands.

There was no carbon monoxide detector at the center.

A wall in a perk rom and in the outdoor playground were observed with peeling paint in an area accessible to children.

Two walls in a toddler room were not maintained as chipping paint was observed accessible to children. A 1 inch wide hole on a wall was also observed in the same room.

A caregiver was missing training in pediatric first aid with rescue breathing and choking.
